When my mother went into rehabilation center after her stroke at 75 years old, I was overwhelmed by everything that needed to be coordinated for her care.
To make matters worse, I felt like I couldn’t sleep at night wondering if she was getting the right care at the right time.
I know her facility was doing their best, but the person in the bed wasn’t another number. It was my mom.
It became crystal clear that I couldn’t do it on my own, and yet finding help I trusted felt impossible.

Daily Routine Support
During our visits, we observe whether your loved one's daily routine is being supported as outlined in their care plan and personal preferences. This may include monitoring participation in meals, therapy sessions, recreational activities, personal care routines, mobility opportunities, and scheduled appointments. We also observe changes in mood, engagement, and overall well-being that may affect their day-to-day experience.
Our goal is not to replace the care team, but to provide an additional layer of advocacy and oversight by identifying concerns, celebrating positive outcomes, and keeping families informed through objective observations and timely communication.

Professional Guardianship Services
When an adult is no longer able to safely make personal, medical, or financial decisions due to illness, injury, disability, or cognitive decline, a court may appoint a guardian to act in that individual's best interest.
Every guardianship is unique, and the authority granted to a guardian is defined by the court. NDTSoulz carries out these responsibilities with transparency, accountability, and a commitment to acting in the ward's best interests.

Your loved one will be visited by a Texas Certified Guardian with more than 11 years of experience with people who are elderly, disabled, neurodivergent and with intellectual disabilities.

We evaluated resident conditions, physical wellbeing, behaviors, nutrition, surroundings, safety, interactions with staff and, most of all, red flags which should be brought to their care team’s attention.

All of our services are tailored to your loved one’s need, including in-person visits, coordination of care and communication, independent oversight of appointments and treatment, adherence to facility schedule and much more.

Visits with your loved one are typically a minimum of 3 hours long, depending on the care plan you select. Additional visits, tailored options and special requests can be updated at any time.

We communicate by phone, text and email depending on the nature of the message. We’re here for you any time for quick questions, concerns and thoughts. You will also get your visit report emailed to you after every visit.

We will provide you with a detailed report on our visit and observations within 24 hours of the visit ending, but will always reach out to your immediately if we see something that requires your attention.

In addition to external factors like cleanliness of their environment or quality of medical care, we pay close attention to subtle emotional and behavioral cues that can be overlooked by busy staff.
